Clinical and Hematological Pictures in Diffuse Peritonitis due to Ileal Perforation in Salmonellosis.



Abstract
A clinical and hematological review was made of 91 patient who had diffuse perito-nitis due to typhoid perforation and were admitted and operated Daegu Fatima Hosp-ital from 1965 to 1974.
The result are summarized as follows.
(1) The age incidence of the disease was highest in third decade and the sex incidence revealed the preponderance of male to female as 2. 25 : 1.
(2) The monthly distributions of patients revealed high incidences in June, July and August.
(3) Pneumoperitoneum was noted on 75.82% of patients.
(4) An overall 21. 98% of patients revealed leucopenia while the rate of leucopenia increased along the time lapse after perforation.
(5) The average rate of leucocytosis was 30.77% and showed prominent increase in leucocytosis over 72 hours after perforation.
(6) The differential count of peripheral blood had the tendency of increasing number of patients who had polymorphonuclear leucocytosis, lymphopenia and eosinopenia.
(7) The Widal¢¥s test revealed positive (over 1:160)in 50%
(8) A single perforation was found in 76. 92% of patients and the rest had 2 to 4 perf-orations. The nearer the ileum to the ileocecal junction, the more frequent incidences of perforations were noted,
(9) High complication rates were noted. Among the frequent complications, wound infection was the highest(43.9%)and the next was wound disruption(21.98%).
(10) The average hospital dates were 24.86 days.
(11) The overall postoperative mortality was 13.19¡Æ0. The mortality rate was higher in leucopenic patients than patients who had normal leucocyte count. The higher mo-rtality rate was also observed in anemic patients and patients who had the longer time lapse after perforation.
